Therapy activities include individual, couples and family interventions related to the specific presenting illness. Responsibilities Develops and implements individualized treatment approach to meet the needs of each patient and family. Works in close collaboration with all members of the treatment team including medical doctors, to further their understanding of significant social and emotional factors contributing to the patient's well-being. Identifies appropriate outside resources, such as community agencies, to assist patients with resuming life in their own community and how to live with the patient's limitations. Provide health education services to patients and their caregivers base on physician orders. Minimum Qualifications Master's Degree in Social Work from an accredited school by the council on Social Work; current licensure to practice as a Certified Social Worker in the State of Utah. Demonstrated human relations and effective verbal and written communication skills; and proven ability to work in a multidisciplinary team, problem solve and resolve conflicts for individuals, couples and groups. Preferences Experience in working with dementia patients is preferred. Experience in working with older adults with chronic health conditions is preferred.
